The following provides a summary of professional fees and billing practices. In addition to the fees described below, you may be additionally charged for other professional services you may require, such as letter and/or report writing, treatment summaries, travel to and from an off-site location, telephone conversations requested or initiated by you, responding to email, attendance at meetings or consultations that you have authorized, or other services you may request.

Professional Practices and Policies

• Individual Adult Psychotherapy session billed at $200 per 55-minute
• Child/Teen Psychotherapy Session $175 per 55-minute
• Family Therapy appointments are billed at $250 per 55-minute hour
• Child/Teen Psychological Evaluations billed at $200 to $250 per hour
• Individual Consultations are billed at $175 per 55-minute hour
• Parent Coaching is billed at $250 per 55-minute hour
• Group Consultations billed variably based on the number of participants

Payment Policy and Billing Practices

I/Road To Healing Counseling ask that you pay at the time of service for evaluations and therapy. Payment by check or credit card is accepted. Your provided credit card information is kept on file.

Some services provided include, but are not limited to: school observation and consultation, consultation with specialists, parent training, and behavioral management and intervention. These will be billed directly to the responsible party.

Procedure for Appointment Cancellations

It is the responsibility of the patient or parent/guardian of the patient to notify Road To Healing Counseling if it is necessary to cancel an appointment. The following conditions apply:

1. No charge will occur, if you notify Road To Healing Counseling 48 hours before the appointment
2. Same day cancellation will result in a full fee charge
3. Two (2) missed appointments without notification – your appointment time will be offered to those on the waiting list

Insurance Reimbursement

If you have a health insurance policy, it may provide some coverage for mental health treatment for out-of-network providers. An invoice will be provided to you for you to submit to your insurance company for reimbursement.

It is the responsibility of the parent/guardian to contact his or her insurance company to verify eligibility of benefits and to find out exactly what mental health or medical services the insurance policy covers prior to the first appointment. Regardless, you are ultimately responsible for payment at the time services are rendered. You should also be aware that your contract with your health insurance company requires that I/Road To Healing Counseling provide the agency with information relevant to the services provided to you if they reimburse you for services. We are required to provide a clinical diagnosis. Sometimes we are required to provide additional clinical information, such as treatment plans or summaries, or copies of your or your child’s entire Clinical Record. In such situations, every effort is made to release only the minimum information about you or your child necessary for the purpose requested. A copy of any report submitted will be provided upon request. By signing the Authorization to Disclose Protected Health Information Form, you agree that requested information is provided to your carrier.

Overdue Accounts

You are responsible for your account and are expected to pay for all services you receive at the time of service. In the case of minor children or adults under the care of a legally appointed guardian, the parent or guardian who brings the patient for treatment is responsible for payment.

You have 30 days to pay your account balance in full. If there is no attempt at payment within 90 days of service, your account will be sent to a collection agency. Patient or responsible party will be held accountable for attorney fees, court costs and collection fees assessed if the account becomes delinquent and is placed with a collection agency.
